Licensed Practical Nurse (LPN) - Home Care - - - -Enjoy -independence, autonomy, and flexibility -in the field to visit patients one on one in their home environment.Hours: Full-time - Days 8:00am - 5:00pm - .8 FTE (32 hours) Fri/Sat/Sun/Mon OR 1.0 FTE (40 hours) Thurs/Fri/Sat/Sun/Mon -Enjoy some of these notable benefits: - -

    • Current license as a Licensed Practical Nurse (LPN) in the State of MN. - -License is active and in good standing.
    • 2 years' experience -as an LPN.
    • Able to travel within geographic area serviced by the office. -Valid driver's -license, reliable transportation, and proof of auto insurance required.
    • CPR Certification -Some functions of what you would do as an LPN:
      • Participate -in ongoing interdisciplinary assessment of the patient.
      • Participate -in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
      • Provides services that are ordered by the physician as indicated in the plan of care.
      • Recognizes and reports changes in the patient's condition to the RN supervisor.
      • Counsels and educates the patient and family in meeting healthcare related needs.
