Job Description:
A company is looking for a Bilingual Care Coordinator LPN to
provide high-quality patient care coordination services.
Key Responsibilities:
Conduct monthly care coordination phone calls with patients and
healthcare providers
Create and revise personalized care plans in collaboration with
healthcare practices
Identify social determinants of health and coordinate referrals and
assistance for patients
Required Qualifications:
At least 1 year of experience as an LPN with active
accreditation
Technology experience with EMR/EHR systems and familiarity with
Microsoft Office and Google Suite
Experience in call center or triage settings is a plus
Experience serving a geriatric population is a plus
Case management or care coordination experience is a plus
